This policy explains what information Tender Hand Healthcare Services (“THHS,” “we,” “us,” “our”) collects through thhs.care, how we use it, and the choices you have. Health information we handle while providing care is protected separately under our HIPAA Notice of Privacy Practices, described below.

1. Who this policy covers

This policy applies to information collected through the THHS website at thhs.care, including its contact forms, referral forms, and employment applications. It describes our practices for visitors, prospective clients and families, support coordinators and case managers, and job applicants.

This policy does not govern the protected health information we collect and use while actually delivering care. That information is handled under our HIPAA Notice of Privacy Practices (see Section 3).

2. Information we collect

Information you give us. When you fill out a form or contact us, we may collect:

• Your name and the name of the person you are inquiring about
• Contact details, such as email address, phone number, and mailing address
• County of residence and the program you are asking about (for example, Supports Program or Community Care Program)
• Details you choose to share about care needs or a referral
• For job applicants: work history, qualifications, availability, languages spoken, and anything in a resume or application you submit

Information collected automatically. Like most websites, we automatically collect limited technical information when you visit, such as your IP address, browser and device type, pages viewed, and referring links. See Section 6 for cookies and analytics.

3. Health information & HIPAA

THHS is a healthcare provider, and much of the information we handle during care is protected health information (PHI) under the federal HIPAA rules and applicable New Jersey law. That information — the records tied to the services we provide to an individual — is governed by our separate HIPAA Notice of Privacy Practices, not by this website policy.

Please avoid sending detailed medical or diagnostic information through website forms or email, which are not secure channels. Share only what is needed to start a conversation, and we will follow up through a secure, appropriate method.

4. How we use your information

We use the information we collect to:

• Respond to inquiries and referrals, and follow up with you or a support coordinator
• Determine program eligibility and coordinate services across the counties we serve
• Review and process employment applications
• Operate, maintain, and improve the website
• Meet legal, regulatory, and DDD program requirements

We do not sell your personal information.

5. How we share information

We share information only as needed to serve you and run our agency, including with:

• Service providers who support our operations (for example, website hosting, form and email tools, and scheduling systems), under agreements that limit their use of the information
• Support coordinators, case managers, and program partners, where you have asked us to coordinate care or a referral
• Government agencies, including the New Jersey Division of Developmental Disabilities, where required to deliver or document services
• Others as required by law, such as in response to a lawful request, or to protect the safety of the people we serve, our staff, or the public

Where sharing involves protected health information, we do so in line with our HIPAA Notice of Privacy Practices.

6. Cookies & website analytics

The website uses cookies and similar technologies to keep the site working, remember basic preferences, and understand how visitors use our pages so we can improve them. To understand site usage, we may use third-party analytics tools, which may set their own cookies and collect standard technical information such as your IP address, browser type, and the pages you visit.

You can control or disable cookies through your browser settings. Turning some cookies off may affect how parts of the site work.

7. How we protect information

We use reasonable administrative, technical, and physical safeguards to protect the information we collect. No website or method of transmission is completely secure, so we cannot guarantee absolute security — another reason not to send sensitive health details through website forms or ordinary email.

8. How long we keep information

We keep information only for as long as needed to respond to your inquiry, provide and document services, and meet our legal, tax, and DDD program obligations. Retention periods for health and service records are set by applicable federal and New Jersey law and by our internal records-retention policy. When information is no longer needed for these purposes, we dispose of it securely.

9. Your choices & rights

You can:

• Ask what personal information we hold about you and request corrections
• Ask us to delete information we are not required to keep
• Opt out of non-essential communications at any time

Your rights regarding protected health information — including access, amendment, and an accounting of disclosures — are described in our HIPAA Notice of Privacy Practices. To make any request, contact us using Section 13.

10. Children & the people we serve

THHS supports individuals with developmental disabilities across the lifespan, and some of the people we serve are minors. When information about a minor or a supported individual is provided, it is typically submitted by a parent, guardian, or authorized support coordinator on their behalf, and we handle it with the same care and confidentiality as all client information.

Our website is intended for adults arranging care or applying for work. It is not directed to children under 13, and we do not knowingly collect their information directly through the site.

11. Links to other sites

Our website may link to other sites, including government resources and our related entity THIC Consulting. We are not responsible for the privacy practices of sites we do not operate. Please review their policies before sharing information with them.

12. Changes to this policy

We may update this policy from time to time. When we do, we will revise the “Last updated” date above. Significant changes will be noted on this page. Please check back periodically.
