Information we collect<\/h2>\r\n    <p><strong>Information you give us.<\/strong> When you fill out a form or contact us, we may collect:<\/p>\r\n    <ul>\r\n      <li>Your name and the name of the person you are inquiring about<\/li>\r\n      <li>Contact details, such as email address, phone number, and mailing address<\/li>\r\n      <li>County of residence and the program you are asking about (for example, Supports Program or Community Care Program)<\/li>\r\n      <li>Details you choose to share about care needs or a referral<\/li>\r\n      <li>For job applicants: work history, qualifications, availability, languages spoken, and anything in a resume or application you submit<\/li>\r\n    <\/ul>\r\n    <p><strong>Information collected automatically.<\/strong> Like most websites, we automatically collect limited technical information when you visit, such as your IP address, browser and device type, pages viewed, and referring links. See Section 6 for cookies and analytics.<\/p>\r\n\r\n    <h2 id=\"health\">3. Health information &amp; HIPAA<\/h2>\r\n    <p>\r\n      THHS is a healthcare provider, and much of the information we handle during care is\r\n      <strong>protected health information (PHI)<\/strong> under the federal HIPAA rules and applicable\r\n      New Jersey law. That information &mdash; the records tied to the services we provide to an\r\n      individual &mdash; is governed by our separate <strong>HIPAA Notice of Privacy Practices<\/strong>,\r\n      not by this website policy.\r\n    <\/p>\r\n    <p>\r\n      Please avoid sending detailed medical or diagnostic information through website forms or email,\r\n      which are not secure channels. Cookies &amp; website analytics<\/h2>\r\n    <p>\r\n      The website uses cookies and similar technologies to keep the site working, remember basic\r\n      preferences, and understand how visitors use our pages so we can improve them.
